Dear Athlete and Parent,
Welcome to Bellevue University and Bruin athletics. Soon, you will be starting a new season at Bellevue University. This letter is to bring you up to date with the latest in the athletic training room.
At Bellevue University, we do not perform any drug testing. Although, athletes may be required by the NAIA during post season to drug test. Also, if we suspect an athlete of illegal drug use during practices, rehab, or treatment, athlete may be questioned or have coaches and or AD made aware of concern.
At Bellevue University, the athletic training department utilizes Sportsware as our online database. All athletes will be required to make a profile and provide information about themselves, such as local address and emergency contact information in case athletic trainer needs to contact someone because of a serious injury or illness. All athletes are required to have health insurance in order to participate in athletics at Bellevue University. There are also a few forms that need to be read and signed that provide crucial information to being an athlete at Bellevue University.
Below are directions on how to make a Sportsware account and progress through the information on Sportsware.
IMPORTANT ACCIDENT INSURANCE INFORMATION AND CLAIM FILING INFORMATION
The following should be helpful in providing all responsible parties with the information necessary regarding the University’s secondary insurance plan and the procedures to follow after an injury.
The University requires that all athletes participating in intercollegiate sports be covered by a primary insurance plan before they are allowed to participate in such sports. Proof of such coverage will be required. In addition to your own primary insurance plan, Bellevue University also carries a secondary accident insurance policy. This policy provides secondary coverage on any medical bills arising from an accident which caused injury while competing in athletics at Bellevue University, but only after a $500 deductible has been met. This deductible is considered “reducing,” which means that when out of pockets are incurred by the member (for a covered expense under the Bellevue plan), those charges will simultaneously be considered towards the school’s deductible as well.
As an accidental insurance policy, it should be understood that the Bellevue University secondary insurance carrier determines whether an injury related to athletics is “accidental.” Pre-existing conditions are not covered; therefore, it is imperative that all injuries are immediately reported to any of the athletic trainers for accurate injury assessment, physician referral (if necessary) and appropriate documentation.
Bellevue’s secondary coverage has a max $35,000 coverage. If for some reason athlete’s expenses should exceed both their primary coverage and secondary coverage amount, it is athlete’s responsibility to pay the remaining difference. Bellevue’s secondary coverage is only for 2 years from the day of sports-related accident. If for some reason athlete’s care from initial date of injury exceeds 2 years, it is athlete’s responsibility to pay the remaining bills that come after that time limit.
International students are welcome to purchase any insurance coverage they would like, but that plans needs to have coverage for college athletic related injuries. If international students do not know where or what insurance to get, Bellevue University does provide insurance for these students at a cost. Medicare and HMO coverage is not recommended because of limited and specific location of coverage but is acceptable. However, we do recommend purchasing additional coverage for college athletic accidents and/or switching Medicare to Nebraska for athlete, in order to provide athlete with the best care possible.
PROCEDURES FOLLOWING INJURY
- Notify the Certified Athletic Trainer of any injury immediately. Certified athletic trainer will make assessment, appropriate documentation, and physician referral (if necessary).
- The athlete will be seen by the team physician (a medical doctor) on a per referral basis arranged by the Certified Athletic Trainer, if necessary.
- Your primary insurance will be directly billed for all services deemed necessary by the team physician at the time the services are rendered.
- If athlete is referred to a physician where a bill will be generated, the certified athletic training staff will file a claim with the University’s secondary insurance on the behalf of the athlete. If athlete goes and sees a doctor on their own for an athletic related injury without certified athletic trainer’s knowledge or referral, it might be more difficult to get secondary insurance coverage.
- Athlete will be responsible for paying any bills that primary and secondary insurance do not cover.
Bellevue University’s athletic department utilizes the best physicians in the Omaha/Bellevue area to give our athletes excellent care without the delay of appointments or lines, if they become injured. As the forms enclosed are reviewed, you may notice that permission is granted for athletic training department at Bellevue University to render care as needed for the health and well-being of its athletes. This is solely to provide the quickest and best possible care.
All paperwork, including physical, concussion testing, Sportsware information, insurance information and forms, must be completed before athlete begins any sport related activities.

For Athletes
Steps to log in on Sportsware for NEW ATHLETES.
- Open browser and type in swol123.net
- Click Athlete/Parent login
- In School ID, type BUBruins. (You will have to be accepted once you have logged in; once accepted by ATC you can continue with setting up rest of your sportsware)
- Fill out all information – General, Address, Emergency contact info, Insurance (make sure to upload pic of front and back of insurance card), and Medical (allergies, medications, immunizations, conditions, etc.)
- Under PHQ-9 tab, fill out completely.
- Under MEDICAL HISTORY tab, fill out medical history
- Under FORMS tab, click and open Student-Athlete handbook (and sign), Consent form (and sign), Drug testing form (and sign). Read Carfully. *When signing* open and click save/validate. On top of screen should be a continue to signing, fill out electronic signature and click sign and complete after you have read forms.
- Take a picture of your physical. Under FORMS tab click add/+ button. Upload physical and rename to Physical and the year.
- We will send Concussion Baseline test from IMPACT to your email address. Click the link and complete the baseline test.
Steps for Sportsware for RETURNING ATHLETES.
- Update any new information under General, Emergency contact info, Insurance (make sure to upload pic of front and back of insurance card, if new), and Medical (allergies, medications, immunizations, conditions, etc.)
- Under FORMS tab, read carefully and resign required forms - Student-Athlete handbook, Consent form, and Drug testing form. *When signing* open and click save/validate. On top of screen should be a continue to signing, fill out electronic signature and click sign and complete after you have read forms.
- Upload new physical under FORMS tab. Take a picture of your physical. When uploading physical, click add/+ button and rename to Physical and the year.
- Fill out new PHQ-9.
- Fill out new MEDICAL HISTORY.
